Add Drop Form Students enrolled at the School of Public Health
During any registration period when students are unable to add or drop online. This form is used to be placed on a class waitlist.
Nondegree Student Reg Form Spring 2010 AVAILABLE 9:00 AM 11/23
Non-degree and cross registering students must submit this form to begin the registration process for Spring 2010 SPH classes
Between November 30, 2009 and January 4, 2010.
Directed Study/Research Proposal Form BUSPH students enrolling in a Directed Study or Directed Research
During any registration period
Withdraw or LOA Form 2009-2010 All BUSPH degree candidates or non-degree students students taking a leave of absence in fall or spring or withdrawing after registering in any semester
Ideal is before first day of target semester. Financial and academic implications if requested after the target semester begins.

Anytime - it is recommended that students make any planned concentration changes prior to the relevant registration period.
Request for Transfer Credit or Waiver BUSPH degree candidates matriculating before Fall 2009
At least four weeks before you need the transfer credit to be approved/pre-approved

Medical Insurance Waiver (See Student Accounting Services for waiver guidelines and to download waiver) BUSPH degree candidates

End of January for Spring 2010.
Official transcripts Are available through the University Registrar at 881 Comonwealth Avenue. There is a fee for this service. Visit the University Registrar's Office at http://www.bu.edu/reg/grades/ordergrades.html for more information.

For an UNOFFICIAL transcript, you may log into the Student Link.  If you cannot access the Student Link, fax a written request, including your full name, student ID number and signature to the SPH Registrar's Office at 617-638-5060.
Any BUSPH past or present student.

HL Core Request for Exemption non U.S. citizens who are MPH students
First five weeks of the first semester of studies
Request to Remediate a Core Course Degree candidates at BUSPH who did not meet mandiatory minimum grade requirements
After the semester ends and before start of upcoming semester
Request for an Incomplete BUSPH degree candidates The signed form must be received before the end of the target term.
Graduation Applications

Applications for graduation are available on the shelves in front of the BUSPH Registrar's Office, Talbot 2C. In most circumstances, electronic version of the application for graduation are not available. However, if you are no longer in the Boston area, you may e-mail This e-mail address is being protected from spambots. You need JavaScript enabled to view it and she will e-mail you the form. Students who are in the Boston area but are unable to make it to the BUSPH Registrar's Office may ask that an application for graduation is mailed to them via USPS. 

Completed applications for graduation must be submitted with the corresponding Graduation Audit Sheet.
Completed applications and corresponding self-audit are due by the published application for graduation deadlines for January, May and September.
