The Forms Who Should Use Them When to Submit
Wait list Form (web form): the Summer 2012 form is active.  Click here.
Students who are closed out of a class during the registration period due to seat reservations or seat capacity being reached. Forms submitted before registration starts will be deleted.
Nondegree Student Registration Form for summer 2012.  Forms submitted before 9:00 AM on March 26 will be deleted.

Non-degree students and Boston Consortium students who wish to take a fall course at BU SPH.
Will be available on March 26.  The deadline is May 1.

Add/Drop Form Students enrolled at the School of Public Health
During any registration period when students are unable to add or drop online.  
Directed Study/Research Proposal Form BUSPH students enrolling in a Directed Study or Directed Research
During any registration period
Withdrawal or LOA Form 2012-2013 All BUSPH degree candidates or non-degree students students taking a leave of absence in fall or spring or withdrawing after registering in any semester By August 31 for fall 2012 & January 15 for spring 2013. Financial and academic implications if requested after the target semester begins.

Students should make concentration changes prior to the relevant registration period.
Transfer Credit & Waiver Policy and Form 
BUSPH Degree candidates matriculating Fall 2009 or later. Call Registrar's Office for more information.
At least four weeks before you need approval.
Medical Insurance Waiver (See Student Accounting Services for waiver guidelines and to download waiver) BUSPH degree candidates

Official transcripts Are available through the University Registrar at 881 Comonwealth Avenue. There is a fee for this service. Visit the University Registrar's Office at http://www.bu.edu/reg/grades/ordergrades.html for more information.

For an UNOFFICIAL transcript, you may log into the Student Link. If you cannot access the Student Link, fax a written request, including your full name, student ID number and signature to the SPH Registrar's Office at 617-638-5060.

HL Core Request for Exemption non U.S. citizens who are MPH students
First five weeks of the first semester of studies
Request to Remediate a Core Course Degree candidates at SPH matriculated into their current degree program in spring 2011 or earlier who did not meet mandiatory minimum grade requirements
After the semester ends and before start of upcoming semester
Graduation Applications for May

Graduation applications and surveys for the May graduation are available on the shelves in front of the BUSPH Registrar's Office, Talbot 2C.  Paper form only!

Completed applications for graduation from single-degree MPH candidates must be submitted with the corresponding Graduation Audit Sheet.
